MARS ride waiver

We've prepared a simple waiver form that we'd like all ride leaders (i.e. the person who proposes the ride) to make use of. Just download it to your computer, then print a copy to bring with you to rides (don't forget to bring a pen, too!). You only need to download it once.

Here's why we're doing this:
1. Our insurance from the League of American Bicyclists (LAB) wants us to make a "good faith" effort to get everyone to sign a release.
2. It gives the ride leader an accurate head count.
3. If the worst happened and someone had to go to the emergency room, each rider will have given permission by signing for the ride leader to have him/her treated if he/she is not conscious.

Being a ride leader doesn't expose you to any special liability, so don't worry about the legal implications of this. Also, we respect the privacy of our members and do not share any information provided on the form.

To download the form, just click on the link of your choice and the download should happen automatically. PC users may need to right-click on the link in order to save the file to disk (depends on your individual set-up).
